Reporting an incident

An incident or breach of the Leaders' code of conduct shall be reported to the Minister, a Church Councillor or Elder or member of the Safe Church Team.

The report shall record the following:

• Name of person reporting the incident
• Name(s) of other persons concerned.
• Date and time of incident
• The Minister or Church Councillor or Elder or member of the Safe Church team to whom you reported the incident
• Describe your concerns, what was observed or what was said.
